
Unlike many medical providers today, we will file directly with your insurance company and bill you for the balance only after your insurance company has paid or denied the claim. We charge no interest on the accounts due us. The cost to provide this program is high, so we have established the following policy. Sources of Payment --Your treatment may be paid from one or more sources:
Health Insurance
The coverage provided by each insurance plan varies. While we will answer your questions as best we can, your health benefits coordinator and your insurance booklet will be the best sources of information about your specific coverage.
Upon verification of coverage, we may agree to bill your insurance in lieu of payment at the time of service. However, most insurance plans require a co-payment to be made by you. Co-payment should be paid at each office visit or weekly.
You must also pay any amount charged to your deductible.
Some insurance plans do not cover all of the services or supplies we may provide in the course of your treatment. Those charges are also your responsibility.
When your insurance has paid or denied your claim, or has asked for additional information from you, we will bill you for the balance due. We ask that you pay promptly or call us to set up a payment plan.
If your insurance has not paid us within 60 days or our filing, we will ask you for payment.
If you change insurance and do not notify us, you will be responsible for the balance.
Worker's Compensation
Upon verification of coverage we may accept acknowledgement of your workers’ comp carrier or employer as temporary arrangement for payment in full. We may also ask for full information and assignment of your health insurance. If your workers’ comp claim is later denied, we will then file with your health insurance. You will then be liable for any balance as described above.
If your claim is covered and paid under workers’ compensation, you will not be billed for any amounts not paid by the insurance.
Third Party Claims
We may accept assignment of your rights to receive payment from a third party insurer, such as may become due to you as a result of an auto or other accident. Please understand that these matters are often the subject of dispute by other parties or their insurance company, and we may also ask for assignment of benefits under your health insurance. If after three months the third party insurer has not paid us, we will file on your health insurance. Any unpaid balance is then your responsibility as described above.
If the matter becomes the subject of litigation, we will ask that your attorney acknowledge our assignment. We will then defer payment of the account in full until settlement. However, due to the cost of carrying these balances until settle-ment, we expect a minimum monthly payment of $25.00 until we are paid from the settlement or your payment in full, whichever is first.
Self Pay
If you are uninsured or your benefits have been exhausted, we will work out a satisfactory payment plan with you. Please ask, especially if you have any extraordinary circumstances. We simply ask for regular payments or that you otherwise stay in touch.
Collections Procedures
We will bill you monthly after insurance has settled, and we ask for and expect payment in full, an installment payment of the greater of 10% or $25, or some communication in response to each billing.
If we have not heard from you within 90 days, we may refer the account to an outside agency. Please help us by keeping communication open.
